适应症
适用于需要补充谷氨酰胺患者的肠外营养,包括处于分解代谢和高代谢状况的患者。
用法用量
本品是一种高浓度溶液,不可直接输注。在输注前,必须与可配伍的氨基酸溶液或含有氨基酸的输液相混合,然后与载体溶液一起输注。1体积的本品应与至少5体积的载体溶液混合(例如:100ml本品应加入至少500ml载体溶液),混合液体中本品的最大浓度不应超过3.5%。
剂量根据分解代谢的程度和氨基酸的需要量而定。胃肠外营养每天供给氨基酸的最大剂量为2克/KG体重,通过本品供给的氨基酸和谷氨酰胺量应计算在内,通过本品供给的氨基酸量不应超过全部氨基酸供给量的20%。

特点
谷氨酰胺是人体中含量最多的氨基酸,具有重要的生理作用。谷氨酰胺是生物合成核酸的必需前体物质,是蛋白质合成与分解调节物,谷氨酰胺在维持肠道机能,促进免疫机能,维持体内酸碱平衡及提高机体对应激的适应等方面都发挥着及其重要的作用,是L-谷氨酰胺的γ-羧基酰胺化物为条件必需性氨基酸。约占人体全部游离氨基酸的61%,是人体内含量最丰富的氨基酸。它既是重要的生化试剂,又是一种很有开发前途的新药。
本品为肠外营养的一个组成部分,N(2)-L-丙氨酰-L-谷氨酰胺可在体内分解为谷氨酰胺和丙氨酸,其特性可经由肠外营养输液补充谷氨酰胺。本双肽分解释放出的氨基酸作为营养物质各自储存在身体的相应部位并随机体的需要进行代谢。对可能出现体内谷氨酰胺耗减的病症,可应用本品进行肠外营养支持。
目前临床上已开始在常规营养支持配方中加入具有上调免疫功能的营养素,如谷氨酰胺、精氨酸和核苷酸等。谷氨酰胺(Gln)是一种条件必需氨基酸,是人体内含量最为丰富的游离氨基酸和重要的氮运载体和供体,而且是体内肠道上皮细胞、淋巴细胞等快速化生细胞的主要能量来源,有助于维持肠道的形态和免疫系统功能,是生物体合成核酸、蛋白质、嘌呤、嘧啶等的重要前体物质,也是蛋白质合成与分解的调节物。但由于Gln水溶性低、在溶液中不稳定,在加热灭菌时可生成有毒的谷氨酸和氨,因而不能直接作为药物,只有将其转化为稳定的衍生物才能充分发挥谷氨酰胺对人体的作用。
